Release and Waiver of Liability
This Release and Waiver of Liability ( the “release”) executed on September 1, 2026 By Artist or Volunteer.
“Artist” or “Volunteer” releases Alpha and Omega Christian Songwriters, a division of 20 West Publishing LLC, A Limited Liability Company organized and existing under the laws of the State of Washington (“Agent”) and each of its directors, officers, employees and agents. Artist or Volunteer understands that the Artist’s or Volunteer’s relationship with Agent is limited to responding to one-time referrals commencing October 24 2026 12:01am and ending October 24 2026 6:01pm, and that no compensation is guaranteed in return for services provided by Artist to such referrals; that Agent will not provide any benefits traditionally associated with employment to Artist; and or Volunteer that Artist or Volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Artist’s or Volunteer’s services in response to referrals by the Agent.
1. Waiver and Release: I, the Artist or Volunteer, release and forever discharge and hold harmless Agent and its successors and assigns from any and all liability, claims and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services I provide associated to Agent referral. I understand and acknowledge that this Release discharges Agent from any liability or claim that I may have against Agent with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide as a result of Agent referrals or occurring while I am performing services associated with referrals provided by Agent.
2. Insurance: Further I understand that Agent does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. I expressly waive any such claim for compensation or liability on the part of Agent beyond what may be offered freely by Agent in the event of injury or medical expenses incurred by me.
3. Medical Treatment: I hereby Release and forever discharge Agent from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my performing or traveling to or from a performance.
4. Assumption of Risk: I understand that the activities that Agent may refer to me may include activities that may be hazardous to me including, but not limited to transporting, setting up, playing, tuning instruments, vocalizing, and other activities involved with musical instruments, acoustic and electronic. This includes, but is not limited to: contact with power supply, power cords and connections, contact with sharp objects, falling over gear, ear damage from loud noises, and damage to vocal cords. As an Artist or Volunteer, I hereby expressly assume risk of injury or harm from these activities and release Agent from all liability.
5. Transportation: I further acknowledge that if I drive my own vehicle, or am a passenger in another driver’s private vehicle in connection with an event/function, that Agent’s auto insurance does not cover such a private vehicle. I also understand that the Agent is not held responsible for assuring the safety and reliability of such private transportation or driver, nor for any non-sponsored activities and travel that Artist or Volunteer might choose to participate in before, during or after the Agent referral function, and I therefore accept the risks and responsibilities associated with such private vehicle travel and activities.
6. Photographic Release: I grant and convey to Agent all right, title, and interests in any and all photographs, images, videos, or audio recordings of me or my likeness or voice made by Agent in connection with my providing services to Agent referral. These photographs, images, videos or audio recordings of me or my likeness may be used in group or individual scenes by mutual consent, in reasonable promotional materials.
7. Copyrights: I acknowledge and agree that the Agent does not and will not make claim to any publishers share of copyrights, writers or songwriters share of copyrights in connection with these audio recordings or videos. Agent is not responsible for disputes or litigation arising out of use of Artist’s or Volunteer’s copyright material once it is performed in public.
8. Other: As an Artist or Volunteer, I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Washington and that this Release shall be governed by and interpreted in accordance with the laws of the State of Washington.
I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.
By signing below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.
